AS THE REGISTERED NURSE (RN ) WEEKEND TRIAGE , you'll be under the supervision of the Home Health Supervisor, the Home Health Registered Nurse, utilizing the nursing process of ongoing assessment, planning, intervention, implementation and evaluation, is responsible for identifying patient / family needs and providing quality care / services in accordance with the attending physician's orders, plan of care and Agency policies and procedures. Responsible for performing patient care on an intermittent basis in the patient's place of residence in close coordination with other disciplines and caregivers involved in the patient's care.
Coverage Area : Coalport, Houtzdale - 10 hour shifts
- Fleet vehicles available for eligible employees
- 2026 Subaru Crosstrek
- Fleet Vehicles are permitted to be used for work AND personal use
- All fuel costs for a fleet vehicle (for work AND personal) are covered by PHH
Shift : Friday, Saturday, Sunday 8am-6pm OR Saturday, Sunday Monday 8a-6pm
QUALIFICATIONS :
Be a Registered Nurse with a valid PA Nursing LicensePrevious home health / long term care experience preferredHave ability to work well independently and within a multidisciplinary teamHave valid PA driver's license, current auto coverage and current auto coverageWHAT WE OFFER :
