Job Summary :

In collaboration with the multidisciplinary treatment team, oversees the development, review / revision, of assigned treatment plans, ensuring compliance with all licensing and regulatory bodies. Provides case management duties for the patients of the Arizona State Hospital.

Job Duties :

• Coordination of development, review, and revision of treatment plans in collaboration with the multidisciplinary treatment team, ensuring compliance with all licensing and regulatory body requirements for treatment plans.
• Provides notifications of outside agencies when requested by statue, rule, licensure or certification or policy, and completes associated documentation.
• Assists in case management for coordination of patient admissions and discharges, to and from the hospital. This includes, but is not limited to the following : contacting outpatient providers, completing related hospital forms, and procuring releases of information.
• Provides department support according to level of education and credentials (i.e. : psycho-educational family group, case management, data collection, administrative tasks, education program).
• Other duties as assigned as related to the position (typically 5% - 10%)

Selective Preference(s) :

Candidate will have a Bachelor's Degree in Psychology, Social Work, or related field.

Preference given to applicants with Public Health experience.

Pre-Employment Requirements :

A State Hospital employee must possess a valid level one fingerprint clearance card issues pursuant to A.R.S.41-1758.07 or must apply for a level one fingerprint clearance card within seven working days after beginning employment.

Retirement :

To help you build a financially secure future, the State makes monthly contributions to finance your retirement benefit. The State will make a contribution to the ASRS in an amount equal to your contribution. In other words, you and the State will each pay 50% of the total cost of the benefit.
