Job Overview

The Respiratory Care Coordination Specialist is responsible for facilitating and managing respiratory care coordination of designated populations of the most complex pediatric patients within an inpatient / ambulatory environment. Leads care team partners in the delivery of high-quality, safe, personalized, and cost-effective, respiratory care outcomes-based care. Develops, implements, and evaluates pulmonary, technology dependent, and tracheostomy education programs. Coordinates and teaches courses to patients, families, and healthcare professionals. Proactively supports efforts that ensure delivery of safe respiratory care while promoting a safe environment at Children's Hospital of Colorado.

ESSENTIAL FUNCTIONS

An employee in this position may be called upon to do any or all the following essential functions. These examples do not include all the functions which the employee may be expected to perform.

Develops and implements specific education programs in accordance with accepted teaching standards such as technology dependent, tracheostomy dependent, complex care, neuromuscular disease, sleep disorders, aerodigestive, and cystic fibrosis patient, caregiver education and other complex care education.

• Collaborates with patients, families, and providers to establish an effective plan of care based on mutually developed goals and in an environment that fosters shared decision making.
• Assesses patient's Respiratory Care needs in the hospital and / or in preparation for discharge to home.
• Monitors patients' adherence to plan of care and progress toward goals. Facilitates and documents changes and / or interventions as needed.
• Provides education and coaching to patients and families to improve health literacy and work towards self-management of their health.
• Creates ongoing processes for patients / families to determine and request the level of care coordination support they desire over time.
• Facilitates patient access to appropriate medical and specialty providers as well as other care coordination support, including, but not limited to, medication acquisition and management, procedures / tests, appointments, and referrals.
• Serve as a point of contact, advocate, navigator, and informational resource for patient, family, care team, payers, and community resources. Facilitates related meetings as needed.
• Provides mentoring / coaching to other care coordination team members within the Respiratory scope of proactive.
• Participates in practice transformation efforts that support ongoing coordination of care for medically complex patients and families through sharing knowledge, acting as role models, and providing technical assistance.
• Participates in the development and revision of policies and procedures, as well as with quality improvement and research activities impacting care for designated patient populations.
• Utilizes information available within the organization in conjunction with technology to support communication, clinical decision making, error prevention, and overall care coordination.

Monitors patient / caregiver comprehension and compliance and reinforces / re-educates as needed.

Works as a member of a multidisciplinary team to provide comprehensive education as needed to patients and caregivers.

Minimum Qualifications

Education : Associate's degree in Respiratory Therapy or Respiratory Care is required at the time of hire.

Bachelor's degree is Respiratory Care or related field preferred.

Experience : Minimum of three (3) years experience in a pediatric environment.

Certification(s) :

BLS / CPR and PALS certifications required.

Registered Respiratory Therapist (RRT) required

Neonatal / Pediatric Respiratory Care Specialist (RRT-NPS) credential from the National Board of Respiratory Care preferred

Certified Asthma Educator (AE-C) preferred

Certified Pulmonary Function Technologist (CPFT) or higher preferred

Licensure(s) : Respiratory Therapist licensure in the state of Colorado is required.

Professional Membership(s) : Active membership in a professional Respiratory Care organization preferred.

Salary Information

Pay is dependent on applicant's relevant experience.

Hourly Range : $34.78 to $52.17
