Acclaim Multi-Specialty Group, Inc. (Acclaim) is a multi specialty medical group practice comprised of over 500 physicians, nurse practitioners, and physician assistants that partner with JPS Health Network (JPS). Job Description : Job Summary :

The Physician Assistant / Total Joint Program Coordinator is responsible for providing and coordinating care for total joint replacement patients. The Physician Assistant / Total Joint Program Coordinator will work alongside and directly support the orthopedic surgeon in the OR, hospital rounds, and clinic.

In addition to assisting with all aspects of patient care, the Total joint Program Coordinator will assist in the planning, development, and implementation of program goals and operations. The Total Joint Program Coordinator coordinates and oversees the ongoing disease-specific certification process to continually improve care for the orthopedic patient population.

Duties and Essential Job Functions :

1. Coordinates the efforts between the surgeons, JPS leadership and staff while tracking the progress of the program and identifying areas requiring attention.

2. Lead efforts to gain and maintain the organization’s DNV Total Hip and Knee Replacement Certification.

3. Responsible for all program management, report generation and review, process improvement, patient and provider satisfaction.

4. Works collaboratively with joint arthroplasty surgeons and anesthesiologists to manage the patient's specialized needs. Coordinates the interdisciplinary approach to providing continuity of care, including utilization management, discharge coordination and planning, and obtaining all authorizations / approvals / transfers as needed for outside services for patients / families.

5. Develops, monitors and evaluates the effectiveness of the joint replacement program care pathway and modifies as necessary.

6. Manages chart and application reviews for quality improvement; analyzes and verifies that data collected by staff is complete, accurate, and consistent.

7. Analyzes the program data with the American Joint Replacement Registry (AJRR) and uses it for quality improvement purposes.

8. Oversees patient education and preparation, resource case management, discharge planning and ensuring patient continuum of care services.

9. Identifies cost-effective measures, makes recommendations for the appropriate utilization of resources and promotes patient self-care management.

10. Works with the interdisciplinary teams to deliver an exceptional patient experience and monitors the quality-of-care delivery.

11. Evaluates and identifies patients’ needs in collaboration with the Orthopedic Patient Care Coordinator.

12. When necessary, interfaces with primary care physicians, specialists, and various disciplines on the development of plan of care. Refers to and supports approved joint replacement protocols.

13. In conjunction with the physician, implements care / treatment plan by coordinating the episode of care for joint replacement surgery patients across multiple providers / disciplines, ensuring patient continuum of care services and monitoring outcomes.

14. Acts as a clinical liaison with outside care providers to include home health nursing, PT, rehab and SNFs, as well as third party administrators.

15. Acts as a patient advocate and educator to assure that the patient has the knowledge to care for his / her condition and patient is educated and empowered to be responsible for participating in the plan of care. Develops individualized patient / family education plan focused on self-management; delivers patient / family education specific to joint replacement program.

16. Participates in data collection and analysis of clinical outcomes of care and customer satisfaction standards.

Knowledge, Skills and Abilities :

  • Must be comfortable working in a large public / county facility
  • Must be able to demonstrate a working knowledge in area assigned, and competency in assessment techniques, diagnosis and treatment processes that apply to the assigned medical specialty area
  • Must have good interpersonal, verbal and oral communication skills to work effectively with other health care professionals and serve as a resource to the community
  • Must be self-directed and take initiative to develop and participate in community activities and medical outreach programs pertaining to the medical specialty area
  • Must be able to demonstrate patient centered / patient valued behavior
  • Must be comfortable working in a large public / county facility

    Required Education and Experience :

  • Master’s degree in Physician Assistant Studies from an accredited program required
  • 2 years of experience working as a PA in Orthopedics preferred
