• Specialty : Family Practice - Primary Care
• Schedule : 1.0 FTE, with 36 Patient Contact Hours; 4 hours of admin time per week. Clinic hours are 7 : 00am to 5 : 00pm. Providers work +2 hours a week (outside of 8 : 00am to 5 : 00pm).
• Call Schedule : On-Call provider takes nightly / weekend / holiday call, 1 : 7.
• Practice Detail : Ascension Medical Group in Chilton, Wisconsin is looking for a Family Practice Physician to join our elite team (2 MDs and 5 Nurse Practitioners-fully staffed). Our clinic is located on the campus of Ascension-Calumet Hospital.

Licensure / Certification / Registration :

Physician MD / DO credentialed from the Minnesota Board of Medical Practice obtained prior to hire date or job transfer date. Licensure is dependent on state associate works in. Verified by local Credentialing Office.
