License : Active VA license required

Certifications : BLS, PALS, ACLS

Provider Must be Local : Only Mileage is Reimbursed - note at the time of name clear

Average Number of Patients Per Day : 18

Number of Exam Rooms : 16

Services Available On-Site : Laboratory and Radiology

Documentation System / EMR : EPIC

Emergency Supplies Available : Defibrillator

Required General Procedures : Evaluation and Mgmt. of Acute Volume / BP Issues

Required Medicine Procedures : Adult Outpatient and Without ICU / CCU

Required Pediatric Procedures : General Outpatient

Required Gynecology Procedures : Pelvic Exam / Pap Smear
