Description

• Conduct thorough evaluations for patients in both inpatient and outpatient settings, crafting personalized treatment plans tailored to their unique needs.
• Implement effective physical therapy techniques focusing on mobility, strength enhancement, balance, and overall functionality.
• Engage in collaborative practices with nurses, physicians, case managers, and other multidisciplinary professionals to achieve optimal patient outcomes.
• Provide education to patients and their families about therapeutic goals, the discharge process, and exercises to practice at home.
• Maintain documentation of assessments, treatments, and progress in accordance with facility and regulatory standards.
