Provide high-quality, patient-centered primary care to patients of all ages, including preventive care, chronic disease management, and acute care.Conduct patient assessments, order diagnostic tests, prescribe medications, and develop individualized treatment plans.Collaborate with a multidisciplinary team of healthcare professionals, including nurses, behavioral health specialists, and case managers.Maintain accurate and timely documentation in the electronic medical record (EMR) system.Participate in quality improvement initiatives and contribute to organizational goals.Educate patients and families about health maintenance, disease prevention, and treatment options.Adhere to FQHC policies, procedures, and guidelines to ensure compliance with federal and state regulations.Qualifications
Board-certified or board-eligible in Family Medicine.Licensed to practice medicine in Indiana or eligibility for licensure.DEA certification.Commitment to serving underserved populations with cultural sensitivity and compassion.Strong communication and interpersonal skills.Bilingual proficiency is a plus but not required.Compensation and Benefits
