Experience : Minimum of 2 years of relevant nursing experience required, including 2 years of post open heart and cardiac care unit experience.

License : Client RN License

Certifications : Basic Life Support (BLS)

Must-Have : Bachelor of Science in Nursing preferred; Associate degree in nursing considered with relevant experience or active matriculation in a BSN program.

Description :

The Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others.

Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care and applies the nursing process based on theory, research, evidence-based practice, and approved organizational nursing standards.

Assesses and evaluates patient care needs while applying critical thinking skills in patient care management.

Integrates relevant assessment and intervention skills in the delivery of nursing care.

Collaborates with an interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care.

Prioritizes all aspects of patient care, including teaching, rounding, coaching, and planning after hospital care, while delegating to others as appropriate.

Educates patients and caregivers by anticipating needs and readiness to learn about their care plan, transition of care, health promotion, and disease prevention.

Communicates effectively and professionally with patients, families, and all members of the Interdisciplinary Patient Care Team.

Manages assignments within the Care Delivery Model and demonstrates effective allocation of resources.

Role models the tenets of Relationship Centered Care through facilitation of various admission, care and discharge processes.

Considers patient values, preferences, cultural diversity, expressed needs, and knowledge in all aspects of care.

Establishes and maintains a therapeutic relationship with patients and families.

Uses evidence-based practices to enhance understanding of patient perceptions of care.

Initiates service recovery for patients and families who have concerns and escalates issues as needed.

Demonstrates caring, respect, compassion, empathy, and active listening through dialogue, body language, and actions.

Supports staff use of evidence-based practices to enhance understanding of patient perceptions of care.

Conducts purposeful hourly rounding on assigned patients addressing key patient needs.

Practices safe and efficient patient-centered care, identifying and escalating barriers and opportunities for improvement.

Demonstrates clinical skills and knowledge crucial to quality and safety in the patient population served.

Implements process improvement strategies based on evidence-based practices and institutional procedures.

Engages in formal and informal peer and institutional review processes.

Collaborates with interdisciplinary teams to create and implement quality improvement projects and evidence-based practice activities.

Contributes to quality and safety practices and compliance measures.

Implements principles of high reliability to provide a safe environment for patients.

Escalates potential safety hazards or gaps from best practice following established protocols.

Utilizes information technology to support quality and performance improvement initiatives.

Performs charge nurse duties as assigned.

Coordinates with leadership to ensure resources are appropriately utilized and inventory suffices for patient care.

Documents accurately and thoroughly in compliance with regulations and standards of care.

Facilitates patient throughput on assigned shifts.

Contributes to the environment of care to support colleagues, patients, their families, and the healthcare team.

Advances clinical competence in nursing practice from novice to expert.
