We're looking for a Certified Nursing Assistant / Certified Medication Technician(CNA / CMT) to join our caring, patient-centered team. In this role, you'll provide personal care and medication support to hospice patients in a way that respects their dignity and enhances their quality of life.

What You'll Do :

• Assist patients with daily personal care needs (bathing, dressing, grooming, hygiene)
• Administer medications safely and accurately per established protocols
• Help with meal preparation, feeding, and hydration
• Support mobility and range-of-motion exercises
• Observe, document, and report changes in patient condition
• Provide companionship, emotional support, and comfort
• Maintain a clean, safe, and respectful environment in the patient's living space
• Follow care plans created by the hospice team
