Job Description

We are seeking a dedicated and detail-oriented  Case Manager  to join our BOE team in Brooklyn. The Case Manager will provide direct support to clients, coordinate services, and ensure compliance with program requirements. This position requires strong organizational skills, excellent communication, and a commitment to helping individuals navigate available resources effectively.

RN Case Manager Key Responsibilities

• Develop and implement individualized care plans, ensuring patient needs are met and goals are achieved.
• Collaborate with healthcare providers, including physicians and social workers, to ensure comprehensive patient care.
• Provide patient education and support to promote behavioral health and self-care.
• Monitor and report patient outcomes, identifying areas for improvement and developing strategies for improvement.
• Maintain accurate and detailed patient records, adhering to medical records and HIPAA regulations.
