Licensed Practical Nurse (Every Other Weekend)

Job Type : Part Time

Location : In Person Fairfield County CT

Pay Range : $40- $50 an hour

Position Overview :

Health Care Connectors is hiring a skilled Licensed Practical Nurse (LPN) to deliver direct care to patients of all ages in a home and outpatient setting. The LPN will support daily clinical functions document care activities and participate in patient education and discharge planning.

Key Responsibilities :

• Provide hands-on nursing care including medication administration and vital sign monitoring
• Assist with specialized procedures and clinical assessments
• Coordinate care plans with the healthcare team
• Document patient care using EHR systems (e.g. WellSky HomeCare HomeBase)
• Educate patients and families on treatment plans and discharge instructions
