Job Summary :
The Registered Nurse will deliver skilled nursing care, case management, and hospice care to individuals facing advancing age, frailty, and serious illness. Responsibilities include patient assessments, medication administration, treatment planning, and interdisciplinary team collaboration. Strong communication and collaboration with patients, providers, and team members are essential.

Responsibilities :
- Conduct patient assessments to determine healthcare needs.
- Administer medications as prescribed by physicians.
- Develop and implement patient treatment plans.
- Collaborate with the interdisciplinary team to coordinate patient care.
- Provide skilled nursing care to patients.
- Manage patient cases to ensure optimal outcomes.
- Offer hospice care and support to patients and their families.
- Maintain accurate and up-to-date patient records.
- Communicate effectively with patients, providers, and other team members.
Required Skills & Certifications :

Shifts between 7 : 00 AM and 10 : 00 AM.Approximately every other to every third weekend from 8 : 00 AM to 5 : 00 PM.Schedule provided a minimum of 7 days prior to the shift, potentially a few weeks prior.Must work the day after Thanksgiving and the days leading up to and after Christmas / New Year.
