About The Position

Job Title : Registered Nurse

Must-Have : Proficient in OASIS and HomeCare HomeBase EMR, used within the last 6 months

Description :

Provides coordinated and directed home nursing care according to physician orders.

Application of the nursing process is required according to agency policies and procedures.

Responsible for supervising and evaluating Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.

Home visits will be a necessary part of the job.

The position requires reliable transportation, a current driver’s license, and agency-required liability insurance.

Expected patient load is between 5-10 patients per day, equating to roughly 6-8 points.

Documentation and travel hold varying point values.

Types of cases include Start of Care, Wound Care, and follow-up visits.

Weekend rotation and on-call duties will be required, varying per location and profession.

Traveling up to 75 miles from the assigned care center is expected.

Mileage will be reimbursed at the IRS standard rate.

All documentation must be completed within 24 hours of the appointment with the patient.

Delayed and incomplete documentation that impacts billing and follow-up patient care will be addressed.

There are no guaranteed hours with this contract.

Clinician must achieve weekly productivity points.

All overtime must be pre-approved.

If patient census is low, there will be opportunities to float to another nearby clinic.

Must be open to on-call rotations.

Local candidates may apply at the same rate.

Candidates must confirm their awareness of driving responsibilities to patients’ homes and the clinic for meetings.

Candidates must confirm their understanding of schedule acceptance requirements regardless of travel distance.

Comfort with driving distances that may exceed 20 minutes between patients is essential.
