Fairview Health Services is seeking an Internal Medicine / Pediatric Physician to join our primary care practice at M Health Fairview Clinics Apple Valley . Join our busy practice which has been in the community for over 20 years.

Practice Details :

• 10 provider primary care practice with more than 5 specialists onsite.
• Patient encounters average 18-22 patients per day.
• 34 patient contact hours per week. Call Frequency : 1 : 10 low intensity - phone consultation only with first tier nurse triage
• Openings : 1.0 FTE (Full Time) - Flexible schedule .
• As a Pioneer Accountable Care Organization (ACO), our physicians use a multidisciplinary team approach in managing clinical care.
• Fully integrated Electronic Medical Record (EPIC).

Apple Valley (Population 50K+) is a southern suburb of Minneapolis. The community has highly rated schools, shopping and many parks as few of the amenities available. Centrally located, this community is minutes from the Mpls / St. Paul International Airport and the Mall of America, with easy access to major freeways and within 30 minutes of the downtown areas of Minneapolis and St. Paul.
