Commitment to providing high-quality pharmaceutical care to underserved populations. Doctor of Pharmacy (Pharm.D.) degree from an accredited institution. Valid pharmacist license in the state of practice. Strong knowledge of pharmacology, drug interactions, and therapeutic protocols. Excellent communication and interpersonal skills. Ability to work effectively in a team-oriented environment. Strong attention to detail and organizational skills. Proficient in using pharmacy management software and technology.
Job Description :
- Dispense medications and provide pharmaceutical care to patients in a Federally Qualified Health Center (FQHC) setting.
- Collaborate with healthcare providers to ensure safe and effective medication use for patients.
- Conduct medication therapy management and medication reconciliation services.
- Educate patients on proper medication use, potential side effects, and drug interactions.
- Maintain accurate and up-to-date patient medication records.
- Participate in quality improvement initiatives related to medication management.
- Stay current with developments in the field of pharmacy practice and adhere to all relevant laws and regulations.
