Position Overview :
The provider delivers comprehensive primary health care, including health promotion and disease prevention, in collaboration with department physicians. The role involves managing a diverse patient population, making evidence-based clinical decisions, and ensuring high-quality, culturally competent care in a collaborative environment.
Key Responsibilities :
- Provide comprehensive primary health care, including health promotion and disease prevention activities
- Collaborate with department physicians and refer patients to consultants as appropriate
- Take medical histories, assess conditions, interpret findings, and make evidence-based clinical decisions
- Maintain timely, thorough, and accurate clinical documentation
- Deliver culturally competent care to a diverse patient population
- Communicate effectively with patients, peers, and stakeholders
- Comply with department and organizational quality measures
- Utilize electronic health records and other computer systems
- Perform other duties as assigned
Physical Examination Responsibilities :
Conduct physical exams and health assessmentsGather and document patient medical historiesAssess and monitor patient conditionsMedical Oversight Responsibilities :
Collaborate with supervising physicians for diagnosis and treatment planningRefer patients to specialists or consultants as neededEnsure compliance with established clinical standards and protocolsRequirements
Qualifications :
Education : Master’s degree from an accredited school of nursing or a physician assistant programLicensure / Certification :Valid APRN or PA license in the state of Connecticut (preferred)Eligible for DEA and state narcotic registrationExperience : 1–2 years’ experience in an ambulatory or urgent care setting preferredLanguage : Spanish language proficiency preferredAdditional Requirements : All candidates must pass a pre-employment background check and drug screen.
