MelroseWakefield Healthcare is a coordinated system of hospitals, physician practices and community-based services providing care for communities throughout north suburban Boston. The Healthy Families Program Home Visitor provides supportive care for identified Healthy Families clients and their families in the home or in the community. The scope of Home Visitor includes assisting with direct care, coordination of care, client/family education, outreach, and advocacy.
