You will promote the use of evidence-based practice through the nursing process, protocols, and procedures. Assess patient's status through interview, observation, and physical assessment of actual problems and outcomes. Develop an individualized plan of care using a collaborative approach with the patient and health care team. Use appropriate interventions identified in the plan of care to achieve expected patient outcomes. Systematically evaluate and document patients' progress toward expected outcomes on an ongoing basis throughout the shift. Modify the plan of care based on evaluation and as needed based on changes in patient condition. Complete required documentation as appropriate, including patient chart and event reports. Demonstrate professional accountability for your own practice by seeking resources and continuing self-development. Demonstrate technical knowledge of equipment and troubleshoot when problems arise. Acquire and maintain current knowledge and skills in evidence-based nursing practice. Promote the professional development of yourself and team members. Demonstrate a practical problem-solving approach to patient moving and handling issues when applicable. Coordinate the interdisciplinary care of patients through collaboration with team members and utilize additional resources when necessary. Analyze clinical situations systematically and act to obtain resources or correct problems to meet or exceed expected outcomes or patient needs. Recognize the need for assistance and utilize resources in a timely manner. Demonstrate ownership and persistence to ensure problems are resolved in the best way possible. Assume the assignment of non-complex and some complex patient management with some turnover in assignments. Consistently prioritize work based on changes in patient condition, changes in assignments, and unanticipated interruptions. Identify and utilize team resources to solve identified problems or needs. Collaborate with the health care team in the organization of tasks and activities for patient care from admission to discharge according to the plan of care.
