We're seeking a Board-Certified or Board-Eligible Physician in Internal Medicine, Family Medicine, or Geriatrics to deliver comprehensive primary care to aging adults across home-based and community settings in San Diego, CA .

What You'll Do

• Serve as the primary care provider for assigned patients, managing preventive, acute, and chronic health needs.
• Perform comprehensive health assessments, order and interpret diagnostics, and guide personalized care plans.
• Collaborate with an interdisciplinary team of Nurse Practitioners, Physician Assistants, and Wellness Coordinators .
• Lead population health initiatives and quality improvement programs that enhance patient outcomes.
• Provide mentorship to mid-level providers and clinical support staff.
• Participate in regular provider meetings and professional development to advance clinical excellence.
