Job Title : Physical Therapist (PT) - Home Health - Full-Time | Brenham, TX

We are seeking a compassionate and skilled Physical Therapist (PT) to join our growing home health care team in Brenham, Texas. Job Responsibilities

• Conduct comprehensive physical therapy evaluations and assessments to determine patients' functional abilities and limitations

• Provide skilled physical therapy treatments in patients' homes, including manual therapy, therapeutic exercises, gait training, and mobility training

• Modify and adjust care plans as needed based on patient progress and changing clinical conditions
• Educate patients and family members on home exercise programs, safety precautions, and techniques to maximize independence
• Collaborate effectively with physicians, nurses, occupational therapists, and other healthcare professionals to coordinate comprehensive patient care
• Maintain compliance with all federal, state, and agency regulations, including Medicare and Medicaid guidelines
• Manage a flexible caseload and schedule while ensuring timely and quality patient care delivery
