Jackson Healthcare Government Services (JHGS) is seeking a dedicated and experienced Registered Nurse (RN) with a strong background in Disease Management to join the healthcare team at David Grant USAF Medical Center , located at Travis Air Force Base, California . As part of one of the largest military medical centers on the West Coast, you will play a critical role in enhancing patient outcomes through the coordination, education, and management of chronic diseases within our active-duty service members, veterans, and their families. Position Details

- Schedule : Monday-Friday, 7 : 30AM-4 : 30PM
- Contract Duration : July 2025 - October 2025
- Healthcare Worker will not be required to work on federally recognized holidays.
Minimum Requirements
Degree : Graduate from a college or universityEducation : Graduate from a college or university accredited by Accreditation Commission for Education in Nursing (ACEN), the Commission on Collegiate Nursing Education (CCNE) or National League for Nursing Accrediting Commission (NLNAC).Certifications in addition to the Basic Life Support (BLS) : Advanced Life Support (ALS) or Advanced Cardiovascular Life Support (ACLS)Experience : 6 years of broad-based clinical nursing experience with 3 years of progressively increasing managed care responsibilities with a focus in Disease Management is highly recommended. Experience with disease management, clinical practice guidelines, performance improvement, and personnel management is strongly preferred. Full time employment in a nursing field for at least 12 of the last 18 months is mandatory. Possess a thorough clinical knowledge and breadth of experience to be able to contribute to population level assessment and management, identify appropriate use of Clinical Practice Guidelines, conduct patient self-care education, and track patient outcomes. Extensive knowledge of Disease Management components : outcomes measurement, collaborative practice, Clinical Practice Guidelines, patient self -management, population identification processes, and process improvement.Licensure : Current, full, active, and unrestricted license to practice as a Registered Nurse as required in the Task Order.Core Duties
Coordinate patient care in collaboration with a wide array of healthcare professionals. Facilitate the achievement of optimal outcomes in relation to clinical care, quality and cost effectiveness.Ensure compliance with standards of care and practice in accordance with all established policies, procedures, and guidelines used in the MTF.Perform physical exam and health histories.Provide health promotions, counseling, and education.Administer medications, wound care, and numerous other personalized interventions.Direct and supervise care provide by other healthcare professionals.Accountable for making patient care assignments based on the scope of practice and skill level of assigned personnel.Recognize adverse signs and symptoms and quickly react in emergency situations.Communicate and collaborate with a diverse group of people for the purpose of informing the healthcare team of plans / actions, for teaching / education to benefit the patient / family and organization.Make referral appointments and arrange specialty care as appropriate.Perform nursing services identified in the Task Order.Conduct research in support of improved practice and patient outcomes.Why Jackson Healthcare Government Services?
