Job Summary : Catholic Charities Archdiocese of New Orleans (CCANO) Homeless Services is seeking an experienced and enthusiastic candidate to take on the role of case manager for our Homeless Services program. The Case Manager will provide immediate case management, housing placement, and supportive social services to homeless individuals and families as they work towards self-sufficiency. CCANO aims to employ a case manager who will aid clients during this transitional period via direct services, advocacy, and empowerment to help to provide stability and promote independence.

Duties / Responsibilities

Essential Job Functions :

  • Assess clients in need of assistance
  • Maintain accurate, well written records and case / service notes
  • Develops, monitors, and regularly evaluates case plans to address concerns and assists clients in becoming self-sufficient
  • Refers clients to appropriate services within CCANO and other organizations
  • Attend meetings with local and regional government agencies and private organizations to secure assistance and / or advocate for clients in need
  • Comply with reporting requirements
  • Adhere to CCANO's policies and procedures
  • Comply with program operational guidelines
  • Other duties as assigned by supervisor

Physical Requirements : Prolonged periods of sitting at a desk and working on a computer. Sitting, Standing, Walking, Talking, sitting at a desk, and working on a computer. Must be able to lift fifteen pounds at times. Physical Demands : Sedentary physical activity performing non-strenuous daily activities of an administrative nature.

  • Stand less than 33% of time on duty.
  • Walk less than 33% of the time on duty.
  • Sit between 33% and 66% of time on duty.
  • Use of hands to finger, handle or feel over 66% of time on duty.
  • Reach with hands or arms less than 33% of time.
  • Full ability to talk and hear.
  • Correctable vision required.
  • Able to lift / carry up to 15 lbs.
  • Minimum Educational and General Qualifications : A bachelor's degree in social work or a related social service field or minimum of three years of work experience in the social service field preferred.

    Strong written, verbal, and interpersonal skills are a must for the position.

    Knowledge, Skills and Abilities :

    This position requires strong organizational, relational, and communication skills, along with the ability to manage multiple projects at a time. The Case Manager will be working in a fast-paced environment and must be able to provide excellent customer service under these circumstances. Proficiency in HMIS and Microsoft Office is essential and preferred. The applicant should have demonstrated the ability to relate well to people of diverse ethnic and economic backgrounds and have commitment to CCANO's mission. Reliable transportation and the ability to be mobile is required throughout employment.
