Police Assisted Diversion (PAD) Registered Nurse

We are seeking Registered Nurse to join our team in our Kensington (Philadelphia, PA) location.

This role is full-time, Monday - Friday 12p - 8p.

Earn $44 / hr.!

Position Details

The RN is responsible for nursing care for individuals receiving treatment / assessment in the PAD location at 265 E Lehigh Ave.

Additional responsibilities will include :

• Provide ongoing patient monitoring, whole person care, administers medications as prescribed and appropriately communicates ongoing patient needs to the medical team as well as all provider / city agencies involved in an individuals care
• Participate in multidisciplinary collaboration and appropriate clinical utilization of a system of care
• Collaborate with all PAD Healthy Village staff to ensure ongoing whole person care occurs; improving an individuals social determinants of health (SDOH)
• Conduct admission assessments through interviews with patients and relatives to gather case history
• Plan and provides nursing care for patients to include wound care and any medical recommendations regarding an individuals care.
• Make nursing diagnosis of the medical status of patients and recommends treatment options.
• Consult with medical and other behavioral healthcare staff in designing appropriate treatment plans for clients
• Complete timely and accurate patient monitoring of vitals and baseline screeners (COWS, CIWA, Falls Assessment, Pain Scale, etc)
• Arrange further medical attention when necessary
• Assist in maintaining a safe and secure environment for staff and patients
• Assist in patient education regarding substance use diagnosis and treatment
• Facilitate patient education sessions on medical topics inclusive but not limited to : HIV, Infectious Disease, MAT options, tobacco cessation, harm reduction
• Document all patient information on medical charts and follows physician's written orders
• Maintain patient confidentiality
