License : Must have a current license

Certifications : CPR, BLS

Must-Have : Proficiency in OASIS and HomeCare HomeBase EMR

Job Description :

• Provide, coordinate, and direct the provision of home nursing care according to physicians' orders based on agency policies and procedures through competent application of the nursing process.
• Responsible for supervising and evaluating Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.
• Conduct home visits and participate in periodic office meetings.
• Must have reliable transportation, a current driver's license, and agency-required liability insurance.
• Average caseload of 5-8 patients per day, translating to 30 points per week.
• Types of cases include start of care, wound care, follow-up visits, etc.
• Required to travel up to 75 miles from the home clinic; mileage reimbursed at the IRS Standard rate.
• Engage in charting and documentation responsibilities, completing all documentation within 24 hours of the appointment.
• Open to on-call rotations and weekend requirements depending on location and profession.
• Must be comfortable accepting all patient assignments and traveling between patient homes.
• Complete retraining if proficiency with HomeCare HomeBase EMR or OASIS was not used in the last 6 months.
• Attend weekly meetings and maintain active communication with the Clinical Manager.
