Salary : $112,000.00 - $135,300.00

Responsibilities

Ascension Illinois is looking for a Board Certified / Board Eligible Advanced Practice Provider, Nurse Practitioner or Physician Assistant for our Family Medicine Department.

This is a great opportunity for the right candidate to partner with Ascension Illinois to provide medical services to patients.

Position Highlights :

100% outpatient

Full-spectrum Family Medicine (kids-adults)

Nice mix of patients and cases

Lab on site

One late evening during the week

1-2 Saturdays per month

Responsibilities :

Provide direct diagnostic, therapeutic, and preventive health care services under the supervision of a physician.

Obtain patient history and perform assessment. Order, perform and interpret diagnostic procedures.

Provide and document direct treatment and management of health conditions.

Provide counseling and education of patients and their families / caregivers concerning preventative health, treatment options and community resources.

Perform preventative health assessments, screening, immunizations and care.

Requirements

Licensure / Certification / Registration :

BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.

Physician Assistant credentialed from the National Commission on Certification of Physician Assistants (NCCPA) obtained prior to hire date or job transfer date required. Credentials are verified by local staff credentialing office.

Advanced Life Support preferred. American Heart Association or American Red Cross accepted.

Education :

• Graduate of accredited Physician Assistant program. Master's degree preferred.
