Shift : 8 : 30 AM - 5 : 00 PM (will be discussed during interview)

Job Summary :

We are seeking a dedicated and experienced Registered Dietitian to join our team at an LTACH (Long-Term Acute Care Hospital) facility in Chicago, IL. This role is responsible for delivering comprehensive nutritional care to patients, including conducting assessments, developing care plans, prescribing nutrition therapies, and collaborating with the interdisciplinary team. The ideal candidate will have strong acute care experience with proficiency in tube feeding and Total Parenteral Nutrition (TPN) .

Key Responsibilities :

• Perform initial and follow-up nutritional assessments for all admitted patients.
• Develop, implement, and update individualized nutrition care plans.
• Prescribe and monitor nutrition therapy to meet patient needs, ensuring tolerance and effectiveness.
• Provide education to patients, families, and staff on nutritional needs and interventions.
• Conduct interviews to obtain dietary history, preferences, and cultural considerations.
• Document all assessments, interventions, and outcomes accurately and in compliance with regulations.
• Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated patient care.
• Partner with the Culinary Services Manager to address patient concerns or dissatisfaction.
• Participate in annual competency testing and maintain required licensure and certifications.
• Stay current with evidence-based guidelines and best practices in clinical nutrition.
