Position Overview : We are seeking skilled and compassionate Nurse Practitioners to conduct home visits in New York’s boroughs, Suffolk, and Nassau Counties. This role offers the opportunity to make a meaningful impact on patient care while enjoying a flexible work environment.

• Conduct comprehensive assessments and evaluations of patients in their homes.
