Position Summary
The LPN provides nursing care to patient and families as prescribed by the physician and under the guidance of the Registered Nurse, in accordance with established policies, procedures and standards. Administers routine services in caring for multiple hospice patients in various settings. Collaborates with, and is part of the clinical team to provide excellence in palliative and end-of-life care.
Duties & Responsibilities :
- In association with other team members, provides high-quality hospice and palliative care for patients and their families.
- Consults and collaborates with other healthcare professionals within the hospice interdisciplinary team.
- Participates in the implementation of patient and family plan of care.
- Contributes to ongoing patient assessment within the LPN scope of practice.
- Accurately documents relevant information in a timely manner.
- Provides care ensuring clinical standards and practices comply with Hawaii Care Choices vision, mission, values, and policies and procedures.
- Ensures safe custody and administration of drugs in accordance with the LPN standards of practice.
- Participates actively in team meetings and QAPI activities.
Skill & Knowledge Requirements :
Ability to handle and administer controlled and prescribed medications in accordance with the Standards of Practice.Ability to coordinate care with the interdisciplinary team.Requires excellent critical thinking, assessment, and active listening skills.Interest and desire to give hospice care with a special sensitivity to the needs of the dying.Have knowledge of, and be sensitive to religious, ethnic and cultural diversity.Must have good communication skills both verbally and non-verbally and the ability to deal with people in crisis in a mature and non-judgmental manner.Consistency in maintaining professional boundaries.Education & Experience Requirements :
Graduate of an accredited practical nurse program.Current LPN (full scope of practice) license in the state of Hawaii.Valid CPR certification.Minimum of one year experience in a healthcare environment or equivalent position.Hours :
Primarily Monday to Friday, 8 : 00 a.m. to 4 : 30 p.m., with rotating of occasional evening, weekend and holiday work with a half-hour meal break.Holidays observed.Upon employment must provide proof of the following :
TB clearance.MMR, Varicella, Hep-B & Tdap vaccinations or titers.Current Hawaii State Driver's License.Pay : $27.00 - $30.00 per hour
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
