Responsibilities
The Value-Based Care Project Manager is responsible for planning coordinating and managing projects that support the organizations Value-Based Care (VBC) Program. This role works with cross-functional teams managing project timelines and ensuring the successful execution of initiatives that improve patient outcomes enhance provider engagement and drive financial performance under value-based contracts.
The Project Manager applies knowledge of value-based care models and healthcare operations to support program design and execution. This position partners with providers payers and internal stakeholders to align priorities monitor progress and facilitate communication to achieve program goals and advance the organizations commitment to patient-centered cost-effective care.
Qualifications
Education
Bachelors Degree in Healthcare Administration Business Public Health Nursing or a related field required
Experience
3 years of project management experience in healthcare preferably within a provider network MSO or health plan setting required
