GENERAL DESCRIPTION :

Act as a team member in a dynamic treatment environment to provide comprehensive evaluation and treatment of adults with substance use including dually diagnosed patients. To assess patients for admission to detoxification and rehabilitation programs and determine appropriate level of care. Provide primary treatment of acute and chronic medical and mental health conditions and triage to higher level of care as needed.

TASK AND RESPONSIBILITIES :

• Conduct intake evaluation with relevant history, health assessment based on the patient's signs & symptoms.
• On an ongoing basis, identify patient needs and make a differential diagnosis based on the clinical presentation, history and clinical findings.
• Develop prioritized, mutually acceptable plans of care according to existing protocols in collaboration with the staff physician. Ongoing collaboration to implement a prioritized treatment plan with appropriate diagnostic tests, pharmacological and non-pharmacological interventions, patient education, and appropriate referrals as needed.
• Provide systematic follow-up of patient progress and presentation, effectiveness of treatment provided, therapeutic processes used by evaluating outcomes and modifying treatments & management plans as necessary to achieve medical, substance use and mental health goals.
• Provide education & information and review risks benefits and alternatives to inform proper consent and as needed to make informed choices and support patient centered care, focusing on health promotion, disease prevention and treatment.
• Participate with other professional colleagues to provide comprehensive physical and mental health care.
• Collaborate with treatment and discharge team to ensure appropriate re-entry into outpatient substance use, mental health and medical treatment
• Support the provision of a safe environment for patients, and promote patient advocacy at the individual and community levels
• In conjunction with interdisciplinary team conduct waived testing for urine, blood work and glucose monitoring.
• Maintain accurate, legible, and confidential medical records utilizing evidence-based documentation.

MINIMUM QUALIFICATIONS :

Current / valid New York State Registered Nurse and Nurse Practitioner (NP) license

SKILLS :
