Welcoming Community Hospital located near Phoenix, AZ has an excellent opportunity for a Physical Therapist . Details are listed below.

Pay : $84,000 - $105,000 / Annually

Physical Therapist Opportunity :

Full-time, direct hire position
Day shift schedule : (4)10 Hour shifts / weekly
Flexible shift options
A full Benefits package with PTO

Physical Therapist Responsibilities :

Performs evaluations, re-evaluations, treatments, and discharges that meet the current standards of the discipline of physical therapy and the hospital.

Documents progress notes, physician communication, and discharge notes for all patients seen

P repares and submits patient charges accurately to provide appropriate billing information

Close-knit team of Therapists work with each other, Physical Therapist Assistants, administrative individuals, and hospital staff to ensure comprehensive, caring treatments

Physical Therapist Qualifications :

Bachelor’s degree in physical therapy or higher

Current AZ Physical Therapist license and ability to obtain an NPI number

BLS Certified
