About The Position
ID : 74371089 Shift : 3x12s, night shift 7pm-7 : 30am, may include some weekend requirements. Description : This role is based at our Hillcrest Resident Care Facility. Position Summary : The Resident Care Coordinator is responsible for the coordination, development, implementation, and evaluation of resident care and clinical practice to ensure a high standard of care is delivered. As a clinical resource and leader for the care team, provides guidance, supervision, and intervention in the delivery of resident care and the care planning process in accordance with all regulations and policies. Minimum Qualifications : Required Montana Licensure (LPN) or Montana Licensure (Registered Nurse) American Heart Association BLS Essential Job Functions : In addition to the essential functions of the job listed below, employees must have on-time completion of all required education as assigned per DNV requirements, Bozeman Health policy, and other registry requirements. Coordinates, supervises, and evaluates the care of residents in a skilled nursing, assisted living, or similar facility. Schedules and trains care staff, and ensures that clinical services are delivered in accordance with regulations and professional standards. Communicates with families and medical professionals as needed, and ensures care plans for residents are properly documented and updated. Provide care and compassion to foster the resident s physical and emotional well-being. Participant in performance improvement activities (such as Fall Risk team or Infection Control Committee) to improve the safety and care of the Residents. The position may necessitate working beyond a standard 40-hour workweek, including weekends and after-hours shifts. On-call work may be required to respond promptly to organizational, patient, or employee needs. Exposures (Continuously) : Bloodborne pathogens, such as blood, bodily fluids, or tissues. Radiation in settings where medical imaging procedures are performed. Various chemicals and medications are used in healthcare settings. Job tasks may involve handling cleaning products, disinfectants, and other substances. Infectious diseases are caused by contact with patients in areas that may have contagious illnesses. Emotionally challenging situations, such as dealing with distressed patients or difficult family interactions.
