Title : Home Care CNA Supervisor

Location : Lafayette, CO

Schedule : Monday-Friday, 8am to 5pm

Pay : $62,788 - $77,176 per year

JOB SUMMARY

Under supervision of the Home Care Services Manager, the CNA Supervisor directs CNA care and services in the community for the PACE participants. Responsibilities include planning of caregiver services, coordination of cares, and scheduling the CNA team for their visits. Works in conjunction with the home care nurses to implement appropriate caregiver services identified as necessary for supporting TRU PACE participants so they can remain living independently in the community for as long as possible.

EDUCATION AND QUALIFICATIONS

• College degree in a field such as rehabilitative or recreational therapy, nursing, social work, or related field, highly preferred. May substitute Certified Nurse's Aide with 2 years relevant experience.
• 2+ years of additional work experience in healthcare
• 1+ years of experience with scheduling responsibilities
• 1+ years of supervisory or management experience
• Knowledge of health care delivery systems, regulations, quality assurance and medical and non-medical needs of the elderly
• Must possess CPR certification or be able to obtain within 60 days of hire
• Must successfully pass agency required background checks and drug screening
