Job Title : RN Coordinator - Infusion
Description
Our client is seeking a dedicated RN Coordinator to join their specialty infusion onboarding / intake team. This is a multi-faceted, in-office role focused on patient onboarding for home infusion services. You'll collaborate with pharmacy, scheduling, nursing, and leadership to ensure seamless care transitions and exceptional patient outcomes.
Patient Onboarding : Assess new patient referrals for suitability, including clinical discernment and home environment evaluation.
Care Coordination : Work closely with pharmacy, schedulers, nursing teams, and leadership to coordinate patient care and ensure all competencies are in place.
Clinical Support : Address patient questions about IV lines, ports, pumps, medication bags, and care logistics.
Whole-Person Care : Develop focused assessments and care plans, considering both clinical and broader patient needs.
Technology Use : Utilize Microsoft Teams (coordination, chat, channels), SharePoint (referral management), and Epic EMR (CPR Plus preferred) for daily operations.
Team Collaboration : Support and teach alongside another RN Coordinator, ensuring patients receive timely and effective care.

Additional Skills & Qualifications
Oregon RN license. BSN preferred, unless extensive infusion experience.
BLS certification (MUST be through American Heart Association).
3+ years of home infusion nursing experience (bedside, clinic, specialty infusion, oncology, dialysis, or emergency room).
Proven experience in care coordination, case management, nurse navigation, hospice, home health infusion, or social work in a hospital setting.
Clinical Discernment : Strong clinical judgment and ability to anticipate patient needs and roadblocks.
Technology Proficiency : Comfortable with Teams, SharePoint, and EMR systems (Epic / CPR Plus).
Excellent communication and interpersonal skills.
Must have demonstrated regular and consistent attendance at work as reported by past employer.

This is a Contract to Hire position based out of Portland, OR.
Pay and Benefits
The pay range for this position is $46.54 - $72.26 / hr.
