Job Overview

The CalAIM Housing Navigator plays a critical role in advancing the agency’s mission by providing housing navigation, tenancy support, and community resource connections to members authorized by Managed Care Plans (MCPs). This position ensures timely, high-quality housing services, collaborates with Continuum of Care (CoC) homeless service systems, and partners with Managed Care Plans (MCPs) and Enhanced Care Management (ECM) providers to support housing stability.

Job Duties (NOT limited to the following)

Conduct outreach and engage clients experiencing housing instability, including those who may be isolated.

Develop individualized housing support plans based on client assessments.

Assist members in identifying barriers to housing and accessing appropriate healthcare, education, and employment resources.

Connect clients with affordable housing options, rental assistance programs, and supportive services.

Educate clients on tenant rights, eviction prevention strategies, and housing-related financial assistance.

Conduct habitability inspections and pre-screen housing units to ensure compliance with Housing Quality Standards (HQS).

Provide landlord-tenant mediation and advocate for clients facing housing discrimination or instability.

Partner with landlords, property managers, and housing authorities to expand housing opportunities.

Coordinate with CoCs, MCPs, ECM providers, and other community organizations to streamline services.

Participate in community outreach, program presentations, and staff training.

Advocate for clients needing SSI, benefits assistance, or emergency housing resources.

Complete intakes, assessments, and HMIS CES surveys in a timely manner.

Maintain accurate documentation in HMIS, Salesforce, and other case management systems per HUD and funding requirements.

Monitor and evaluate clients' housing progression through regular check-ins.

Ensure compliance with agency standards, funding contracts, and local / state housing policies.

Must Have Skills / Qualifications

Bachelor’s degree in social services or related field, or 3 years of experience in case management, housing support, or working with homeless populations.

Knowledge of HUD regulations, Coordinated Entry Systems (CES), and housing rights.

Strong organizational, problem-solving, and client advocacy skills.

Experience in trauma-informed care, harm reduction, and motivational interviewing.

Ability to build partnerships with landlords, realtors, and property management companies.

Familiarity with local housing resources, public benefits, and eviction prevention programs.

Other Requirements :

Valid California driver’s license (if driving for the job) or ability to use public transportation.

Cultural competence and ability to work with diverse communities.

Ability to manage multiple cases, prioritize tasks, and work independently or in a team
