About the Role :

The Certified Nurse Aide (CNA) at Methodist Transitional Care, plays a vital role in delivering compassionate and high-quality care to patients in a transitional care setting. This position involves providing direct patient care, assisting with daily living activities, and supporting the nursing team to ensure patient comfort and safety. The CNA will work closely with interdisciplinary teams to monitor patient conditions and report any changes promptly, contributing to effective care planning and recovery. This role requires dedication to maintaining a clean, safe, and supportive environment for patients during their recovery or rehabilitation phase. Ultimately, the CNA helps enhance patient outcomes by delivering attentive, respectful, and professional care 24 / 7.

Minimum Qualifications :

  • Current and valid CNA certification recognized by the state.
  • High school diploma or equivalent.
  • Completion of a state-approved nurse aide training program.
  • Basic life support (BLS) certification or willingness to obtain upon hire.
  • Ability to pass a background check and drug screening.

Preferred Qualifications :

  • Previous experience working in a transitional care or rehabilitation facility.
  • Familiarity with electronic health record (EHR) systems.
  • Additional certifications such as CPR or first aid.
  • Strong communication skills and ability to work effectively in a team environment.
  • Experience with patient lifting equipment and safe patient handling techniques.
  • Responsibilities :

  • Assist patients with activities of daily living including bathing, dressing, grooming, toileting, and feeding as needed.
  • Monitor and document patients’ vital signs, physical condition, and behavior, reporting any significant changes to nursing staff.
  • Support patient mobility and exercise routines to promote recovery and prevent complications such as bedsores or muscle atrophy.
  • Maintain a clean, safe, and organized patient environment by following infection control protocols and facility policies.
  • Collaborate with nurses, therapists, and other healthcare professionals to ensure coordinated and comprehensive patient care.
  • Provide emotional support and reassurance to patients and their families, fostering a compassionate care atmosphere.
  • Assist with the preparation and transport of patients for medical procedures or therapy sessions as required.
  • Skills :

    The CNA will utilize strong interpersonal and communication skills daily to interact effectively with patients, families, and healthcare team members, ensuring clear and compassionate exchanges. Attention to detail and observational skills are critical for accurately monitoring patient conditions and documenting care activities. Physical stamina and manual dexterity are essential for assisting patients with mobility and personal care tasks safely. Familiarity with healthcare protocols and infection control practices ensures the maintenance of a safe environment for patients and staff. Additionally, proficiency with basic healthcare technology, such as electronic health records, enhances the efficiency and accuracy of patient care documentation.
