We are seeking a Care Coordinator in Santa Rosa, CA. In this role, you will coordinate care by matching clients with caregivers, manage and update caregiver schedules, conduct client intakes and introductions, fill in for last-minute call-offs, collaborate with referral sources, and assist with office tasks, hiring, and on-call duties

Responsibilities :

  • Conduct client intakes and initiate services, including caregiver introductions during first shifts
  • Manage and update caregiver schedules, ensuring all shifts are covered, including filling in for last-minute call-offs
  • Conduct Quality assurance visits and make updates to care plans as needed
  • Use scheduling software to maintain accurate records
  • Handle recruitment and hiring processes as needed
  • Assist with internal and external audits
  • Assist with office tasks and answer phone calls
  • Take on rotational weekend on-call duties
  • Demonstrating a thorough and complete knowledge of the agency including :
  • o our vision, mission and values;
  • o the services we provide; and

    o how we differentiate ourselves from other home care agencies

  • Ensure compliance with policies, procedures, and regulations
  • Perform other duties as required
  • Qualifications :

  • Highschool diploma or equivalent
  • 1+ years of scheduling, administrative, or customer service experience, preferably within a healthcare or home
  • care setting
  • 1+ years of caregiving experience
  • Ability to solve problems quickly and independently
  • Excellent communication skills, both written and verbal, with a compassionate approach to client and caregiver
  • interactions
  • Proficiency in Microsoft Word, Excel, or similar applications

    Compensation : $23.00 per hour
