Job Description
The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an in-person full-time role for an APRN or PA with at least 2 years of experience, preferably in long-term care.
They work independently to manage patients’ medical needs in PALTC settings, aiming to improve care quality, patient outcomes, and reduce hospitalizations and costs.
Responsibilities include conducting assessments, diagnosing, treating, prescribing, and developing care plans; collaborating with facility staff and external providers; ensuring compliance and documentation; and promoting health and safety.
Must demonstrate strong clinical judgment, communication, and interpersonal skills, with proficiency in EHR and telemedicine tools.
Qualifications include relevant degrees, licensure, and board certification (AANP / ANCC / PA). Benefits include health, dental, vision, and retirement plans.
