Role Summary :
This position operates Monday through Friday (may require some weekend hours) and is designed to strengthen participant connection to care, increase engagement in recovery services, and promote stability through ongoing peer-based support and resource linkage.

Core Responsibilities :
1.Provide Peer-Based Support – Establish trust and rapport with clients through authentic, recovery-oriented peer engagement and encouragement.
2.Outreach and Engagement – Conduct outreach to individuals connected with the Morse Clinic Mobile Unit who may be at risk of disengagement or in need of additional support.
3.Support MOUD Participants – Educate and assist participants in understanding and adhering to their MOUD treatment plans, promoting stigma reduction and empowerment.
4.Coordinate Participant Services – Support clients in scheduling intakes, counseling, and medical appointments; maintain communication to ensure follow-through and engagement.
5.Facilitate Linkage to Resources – Connect participants to essential community supports, including healthcare, recovery meetings, housing, employment, and transportation.
6.Advocate for Participant Needs – Help clients navigate barriers in healthcare, housing, employment, and recovery systems while fostering self-advocacy.
7.Collaborate in Care Planning – Participate in interdisciplinary meetings to offer peer perspectives on participant progress and needs.
8.Uphold Ethical and Professional Standards – Maintain accurate documentation, confidentiality, and compliance with all agency and program policies.
9.Bridge Between Systems of Care – Coordinate communication and service connections among Morse Clinic staff, community partners, and treatment programs.
10. Provide Supportive Accompaniment and Transportation – Assist participants in accessing services by transporting or accompanying them to appointments, community resources, and recovery-related activities, ensuring safety, advocacy, and continuity of care.
Work Environment :
Physical Demands :
Language Requirements : English is the primary language of software, documents, participants, and staff of HT, though speaking additional languages is a valuable asset. Must effectively communicate with participants, staff, and external partners. Ability to document services accurately and contribute to team discussions.
Travel Requirements : This position requires regular in town travel between both program campuses and may involve transporting or accompanying participants to appointments, community resources, and partner agencies. The employee must be able to travel independently and support participants in accessing services that promote their recovery and overall well-being.
