Job Title : Nurse Case Manager, Emergency Department Profession : Nursing Specialty : Care Manager, Registered Nurse Duration : 13 weeks Shift : Sunday - Saturday with variable shifts Hours per Shift : 7 : 00 AM - 5 : 30 PM Experience : Minimum of 3 years in care / case management, with 2 years in the Emergency Department and 2 years of patient care experience License : A valid, current, full and unrestricted Professional Nursing License to practice nursing in any state of the United States of America or its territories Certifications : Must have and maintain current Basic Life Support (BLS) certification by a recognized organization. Must obtain Case Manager Certification (CCM or ACM) within 2 years if not already obtained Must-Have : Minimum of 2 years of current care / case management experience in the Emergency Department Proficient with Electronic Medical Records, Outlook, Microsoft Office Word, and Excel Must have 1 year of travel experience Description : The Nurse Case Manager is responsible for establishing, coordinating, and maintaining processes to increase patient throughput to the most appropriate level of care. This position facilitates interdisciplinary care across the continuum for Emergency Department patients. The Nurse Case Manager collaborates with patients and families, multidisciplinary teams, physicians, community resources, and payers to ensure appropriate determination of patient progress and level of care. Essential responsibilities include patient and family assessment, resource management, identifying patients appropriate for different admission statuses, care facilitation, and discharge planning. The Nurse Case Manager collaborates with other case managers, social work staff, and administrative leadership to ensure efficient and cost-effective care. Knowledge of pathophysiology, pharmacology, and clinical care processes is essential for developing clinical practice guidelines. This role evaluates patients in the Emergency Department for medical necessity and care coordination needs. The Nurse Case Manager works with clinicians to propose and implement plans for safe discharge with physician approval. This position interacts with patients of all ages and individuals with mental health issues to help them navigate and access resources for their well-being. Education requirements include an Associate’s degree in Nursing.
