About the Role
As a provider with Matrix, you will conduct in-depth health assessments that can last up to one hour. These assessments allow for a thorough understanding of a member's overall well-being, including their health history, medication compliance, and social factors.
- Perform detailed adult and geriatric assessments, including medical history, diagnosis, treatment, education, referrals, and documentation based on Matrix’s protocols.
- Order and administer necessary point-of-care and laboratory tests.
- Collaborate effectively with other healthcare professionals, such as physicians, case managers, and key caregivers.
- Educate patients and coordinate with Primary Care Physicians (PCPs) for follow-up care.
- Deliver services in the member's residence or skilled nursing facilities.

Master's Degree or relevant experience and completion of NP or PA licensure.Active NP or PA licensure in the state of practice, with prescription authority or the ability to obtain it.Board certification in relevant specialties such as Adult, Family, or Acute Care.Current BLS, ACLS, or CPR certification.Preferred one year of experience as an NP or PA; new graduates are encouraged to apply.Valid driver's license and adequate automobile insurance.Strong computer skills and familiarity with health / medical record software.Excellent verbal and written communication abilities.Our Culture
