About the Role

As a provider with Matrix, you will conduct in-depth health assessments that can last up to one hour. These assessments allow for a thorough understanding of a member's overall well-being, including their health history, medication compliance, and social factors.

• Perform detailed adult and geriatric assessments, including medical history, diagnosis, treatment, education, referrals, and documentation based on Matrix’s protocols.
• Order and administer necessary point-of-care and laboratory tests.
• Collaborate effectively with other healthcare professionals, such as physicians, case managers, and key caregivers.
• Educate patients and coordinate with Primary Care Physicians (PCPs) for follow-up care.
• Deliver services in the member's residence or skilled nursing facilities.
