Position Summary : The Case Manager (CM) works as a member of a multi-disciplinary team within VOA Oregon's outpatient and residential substance use disorder and mental health treatment programs providing case management services to clients. The CM provides direct support to clients to include systems navigation, needs assessment, linkages to community resources including housing, financial, medical, employment, benefits, and other services as needed.

Schedule : This is afull-time position; 40 hrs. per week; Monday thru Friday, with occasional evenings and weekends as needed.
