The primary purpose of the RN Discharge Planner position is to provide ongoing support and expertise through comprehensive assessment, planning, implementation, coordination and overall evaluation / monitoring of individual patient needs. The overall goal of the position is to enhance the quality of patient management and satisfaction, to promote continuity of care and cost effectiveness through the functions and integration of case management, utilization review and management and discharge planning. This position requires collaboration with various departments within the hospital to meet the needs of the patients and their families. Will also correspond with various insurance companies to assure that they are receiving the information they are requesting to approve the patient's admission.

Castleview Hospital maintains a Gold Seal of Approval® from The Joint Commission, is an Accredited Chest Pain Center, a Stroke designated facility, and has been named a Top 100 and Top 20 rural and community hospital several times.

Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.

Must be able to work in a stressful environment and take appropriate action.

Able to operate basic office equipment, fax machine, phone and computer.

Performs admission and discharge screening using MCG guidelines and Two Midnight Rule criteria.

Collaborate with the physician, patient, family, and community partners to develop a safe discharge plan.

Works with insurance companies as needed to approve patients' admission, continued stay and discharge.

Arrange peer to peer reviews as needed

Document appropriately in the electronic health record
