• Provide care to patients of all ages and coordinate care with case managers
• English / Spanish bilingual skills a plus
• Monday through Friday, 8 am to 5 pm schedule, with one late day per week
• Position eligible for sign-on bonus and loan repayment assistance
• Hospital-employed position
• Patient census is 22 - 25 per day
