Licensed Occupational Therapist

Location : Philadelphia, PA (and surrounding areas)

Join a growing home health care team serving Pennsylvanians in Delaware County, Philadelphia County, and Chester County. We are looking for a licensed Occupational Therapist (OT) to provide in-home care .

Responsibilities :

Provide in-home OT evaluations and therapy services for adult and elderly CHC members

Develop personalized treatment plans focusing on ADLs, IADLs, home safety, and functional improvement

Educate patients and caregivers on adaptive techniques, equipment use, and safety strategies

Collaborate with Managed Care Organization (MCO) service coordinators, nurses, PTs, and caregivers

Recommend durable medical equipment (DME) and home modifications when needed

Complete timely documentation in the required system to meet billing and CHC requirements

Attend virtual or in-person case conferences when necessary

Requirements :

Master's or Doctorate degree in Occupational Therapy preferred

Active Pennsylvania OT license

CPR certification

Valid driver’s license, reliable vehicle, and auto insurance

Familiarity with Community HealthChoices , PA Health & Wellness , and / or Keystone First CHC a plus

Comfortable using electronic medical record (EMR) systems a plus

Preferred Qualifications :

Experience with Medicaid / Medicare dual-eligible populations

Prior home health experience strongly preferred

Bilingual (Spanish, Russian, Mandarin, etc.) is a plus

Knowledge of OASIS documentation (if applicable)
