• Conduct regular home visits within your assigned area to evaluate safety, assess physical and cognitive status, and monitor for any changes in health conditions.
• Review medications with patients and caregivers, provide clinical coaching , and ensure adherence to treatment plans and physician recommendations.
• Follow a personalized care plan , delivering ongoing education, coaching, and support through both in-person visits and a digital care platform.
• Build strong, trust-based relationships with patients and their caregivers, promoting engagement and collaboration in care decisions.
• Identify social, emotional, and behavioral health needs, offering referrals to community resources and services where appropriate.
• Document all assessments, interactions, care updates, and interventions in a timely and accurate manner using case management systems.
• Collaborate with primary care providers, specialists, and social workers to coordinate and streamline care delivery across multiple settings.
• Assist caregivers in reducing burnout , improving care confidence, and making informed decisions on behalf of loved ones.
