RN Case Manager

This is a 13-week contract position.

Schedule

8 : 00 AM - 4 : 30 PM, with shifts rotating every other weekend.

Job Description

Selected Candidates will complete their orientation at the designated facility and then transition to different units as required. This role involves working as part of a triad model with an RN / SW / CMA team in a fast-paced, high acuity hospital environment.

Responsibilities

  • Conduct initial and discharge assessments using Interqual criteria.
  • Collaborate with interdisciplinary teams to facilitate comprehensive patient care.
  • Manage complex case loads effectively.

Required Experience / Certifications / Licensure

  • Current RN Florida / Compact license.
  • 2-4 years of experience as a Case Manager in an acute care facility.
  • Experience with Meditech, Midas Nate, Outlook, and Imobile is preferred.
  • HCA experience is highly preferred.
