Primary Care Nurse Practitioner

COMPENSATION : Salary $145k + 15% of base

Transportation is provided to patients that need it at no charge.

Role :

• Compensation : $145k + 15% of base
• Bonus : 15% of base
• 18 patients a day
• Hours : Monday – Friday work schedule; Clinic Hours 7 : 30a – 5 : 00p (last patient is scheduled at 4 : 15)
• Work / Life Balance
• Small Patient Panel – 450 patients’ max
• Administrative time included in schedule
• No nights or weekends
• 100% outpatient
• Dedicated Care and support team (have your own MA / Scheduler /
• Patient Population : Seniors with 3-5 chronic diseases
