FLSA Status : Exempt

Sign-On Bonus : $3,000; no relocation package is currently offered

Location : Home Office is Senior TLC Center - Hickory, NC; supporting their client population

Position Summary : As a Registered Nurse (Home Care), you'll be at the heart of compassionate, high-quality care, delivered directly in patients' homes. You'll assess health conditions, create and manage personalized care plans, and monitor progress to support optimal outcomes. This role involves close collaboration with physicians, therapists, and social workers to ensure holistic, coordinated care. You'll also educate patients and families on managing health conditions and provide emotional support during challenging times, helping individuals maintain independence and improve their quality of life in a familiar setting.

Key Responsibilities :

• Perform comprehensive in-home assessments of patients' physical and mental health
• Develop, implement, and revise individualized nursing care plans based on patient needs and physician directives
• Administer prescribed medications, treatments, and therapies while adhering to safety protocols
• Monitor patient progress and document clinical observations accurately and promptly
• Collaborate with interdisciplinary healthcare teams and family members to coordinate care
• Educate patients and caregivers on disease management, medication use, and health maintenance
• Respond swiftly to changes in patient condition and escalate concerns appropriately
• Ensure compliance with agency policies, healthcare regulations, and infection control standards
